How to refer a patient
- Simply book your patient's appointment by telephoning 01206 766266. Then complete the standard referral form which is part of our information pack and post it to us in the pre-paid envelope provided.
- Alternatively, you could download, complete and post or fax the form to 01206 546541. We will then carry out the necessary arrangements to complete the referral.
